A physical exam form will be requested on the first day of class. If your child's physical exam has not yet been completed, we must have the RCS Head Start Physical Exam Form completed by a physician, certified nurse practitioner, or public health nurse within 30 days, or your child may not be allowed to come to school. If you need assistance in locating a physician, setting an appointment for your child's physical, or transportation, please contact your Family Advocate.
